AHH Youth Ambassador Parent/Guardian Consent & Participation Agreement

Participants under 18 may participate in the AHH Ambassador Program only through an account established, owned, and controlled by a parent or legal guardian who is at least 18 years of age. The adult account holder is the contracting party. The Youth Ambassador does not independently enter into this agreement or any related agreement with AHH.

You consent to the Youth Ambassador's participation in the program, including being identified publicly by first name and last initial only, and you may withdraw that consent at any time by contacting AHH, which will deactivate the youth's public participation.

You agree to supervise the Youth Ambassador's participation, including any in-person activity, event attendance, and online or social-media activity conducted in connection with the program.

AHH does not pay minors. Any commissions, bonuses, or other compensation associated with the account are payable solely to you as the registered adult account holder and financial payee. You are responsible for providing accurate payout information and for any tax reporting, withholding, or filing obligations that arise from compensation you receive, including submission of a Form W-9 where required.

Bracelet sales and awareness activity connected to the account may be attributed to the Youth Ambassador as “Youth Sales/Activity Credit.” Youth Sales/Activity Credit exists for educational tracking, goal setting, recognition, achievement, mentorship, business and financial-literacy learning, personal development, and community-impact tracking.

Youth Sales/Activity Credit is not wages, earnings, or compensation owed to the Youth Ambassador, and it does not make the Youth Ambassador the commission payee. Applicable commissions remain payable to you as the registered adult Parent/Guardian Account Holder.

AHH does not authorize, require, encourage, or instruct Youth Ambassadors to engage in door-to-door sales, canvassing, peddling, visiting homes or businesses, approaching strangers, or any independent in-person solicitation. Training materials describing in-person solicitation are restricted to Adult Ambassadors who are 18 or older and are not provided to Youth Ambassador accounts.

Appropriate Youth Ambassador participation includes:

• Learning about sudden cardiac arrest (SCA) in young athletes.
• Talking about SCA awareness and heart-health screening.
• Sharing the mission with family and friends.
• Attending age-appropriate events with parent or guardian supervision.
• Sharing AHH-approved content online where age-appropriate and with your supervision.

There are no required hours, sales quotas, performance minimums, assigned territories, or requirements to travel to sales locations. Participation is entirely voluntary and may stop at any time without penalty.

Youth Ambassadors may use social-media platforms and online services only where they meet the applicable platform's minimum age requirements and must comply with that platform's terms of service. Parent or guardian supervision is required for Youth Ambassador online participation. AHH does not instruct minors to circumvent platform age requirements.

You are responsible for the Youth Ambassador's safety during any activity connected to the program. Youth Ambassadors should not participate in any activity without adult supervision, should never share personal contact or location information publicly, and should never meet with or travel with people they do not know.

Information collected about the Youth Ambassador is handled in accordance with the AHH Privacy Policy. Additional requirements apply to children under 13. Public pages, leaderboards, and support pages display first name and last initial only. Signature records, consent records, dates of birth, and payout information are never publicly displayed and are accessible only to authorized AHH administrators.

Participation must comply with all applicable federal, state, and local laws, including any laws governing youth work, solicitation, and permits. Parental permission does not authorize conduct that is unlawful. AHH may restrict or end participation at any time for safety, compliance, or program-integrity reasons.

By accepting this agreement you acknowledge that: you are at least 18; you are the parent or legal guardian; you consent to the youth's participation; you are the registered account holder; the youth does not independently contract with AHH; compensation is payable to you; you will supervise participation; minors are not authorized to conduct door-to-door or independent in-person solicitation; and participation will comply with applicable law and AHH policy.
